What information is shared?
Your full name, contact details, city and country, treatment of interest, preferred language and contact method, and a description of your concern. You may attach existing X-rays or photos if you wish. No ID or passport document is requested.
What is it used for?
Your information is used by Melit Dent to review your preliminary assessment request and contact you. This form does not replace a definitive diagnosis, treatment, emergency care or appointment confirmation.
When you submit your request
Your form information and attached files are emailed to the clinic at melitdent@gmail.com. New requests are not permanently stored in the website database or file storage. Information may be processed and stored by email delivery services and in the clinic’s email account.
Optional health information
X-rays, photos inside the mouth and your description may contain health information. Share only the information needed for a preliminary assessment. Do not attach information about other people without their permission. If you are submitting a request for your child, state that you are their parent or legal guardian.
Contact via WhatsApp
WhatsApp links are optional and open a separate service. Information you share through that service is also subject to WhatsApp’s data processing terms. Files submitted to the website are not automatically transferred to WhatsApp.
